The face-to-face consultation is always 100% human. IntakeOS only handles the step before the patient walks in.

This section tells IntakeOS what to ask patients, who qualifies for a consultation, and how to follow up. The more detail you provide here, the better IntakeOS performs for your specific clinic.
Think of the Heartbeat as the knowledge your AI intake coordinator carries about your clinic. It knows your procedures, what questions to ask, what answers mean someone is a good candidate, and when to follow up. You provide this information below β your setup team does everything technical. Nothing goes live until you have reviewed and approved it.
Procedures You Offer
List every procedure your patients inquire about. For each one, tell us the key question that helps determine if someone is a good candidate. For example, for LASIK this might be: "Do you currently wear glasses or contact lenses?" For hair restoration it might be: "How long have you been experiencing hair loss?" These are the questions your coordinator would normally ask over the phone.
How Should Consultations Be Booked?
When a patient qualifies, how do you want IntakeOS to offer them a time? Choose the option that fits your clinic best.
Who Should NOT Be Booked?
Are there any situations where IntakeOS should NOT book a consultation and should instead pass the patient to your staff? For example: patients under 18, patients who are pregnant, patients who have had prior eye surgery (for LASIK), or patients asking about a procedure you don't offer. Write these in plain language β your setup team will translate them into the system's rules.

Attendance Confirmation & No-Show Recovery
Here is how the attendance process works β and why it matters for billing:
β± After every scheduled consultation: IntakeOS waits 2 hours, then sends the person you name below a simple email asking: "Did [Patient Name] attend their consultation today?" You reply with one word β ATTENDED or NOSHOW. If ATTENDED, the $150 success fee is charged to your card on file. If NOSHOW, no charge is made and IntakeOS automatically contacts the patient to offer a new time.

IntakeOS is built on 8 software services. You create an account with each one, your clinic pays each vendor directly (~$200β330 USD/month total), and our setup team connects everything. You own every account.
Each service does a specific job β your phone line, your calendar, your patient emails, your AI voice. They are industry-standard tools used by thousands of businesses. We specify this exact combination because we have built and tested IntakeOS on it β this is how we guarantee the 15-second response time and compliance logging we promised. None of these replace your existing clinical software.
Your IntakeOS system β including the OpenClaw AI operating system, NemoClaw security layer, your patient booking calendar, and your compliance logs β all run on a private server that your clinic owns and pays for directly. This is one of the most important things that makes IntakeOS different from other AI tools: your patient data never sits on someone else's shared cloud. It lives on a server registered in your name, in the correct country for your compliance requirements.
Canadian clinics use OVH Montreal. OVH is a leading cloud provider with a data centre physically located in MontrΓ©al, QuΓ©bec. Because patient data never leaves Canadian soil, your clinic satisfies BC's Personal Information Protection Act (PIPA) and Canada's federal PIPEDA privacy requirements by design. Using a US-hosted server for Canadian patient data would put your clinic in breach of PIPA β this is not optional.
US clinics use LiquidWeb HIPAA-compliant hosting. LiquidWeb operates HIPAA-compliant hosting infrastructure in the United States. Before any patient data is processed, your IntakeOS engineer coordinates the signing of a Business Associate Agreement (BAA) between your clinic and LiquidWeb β a legal requirement under HIPAA for any vendor that handles Protected Health Information.
You create the server account and own it. Your IntakeOS engineer never has ownership β they are given temporary access to configure your system, then that access is revoked at go-live. Here is what you need to do after creating your account:
Step 1: Create your account at OVH Montreal (Canada) or LiquidWeb (US) using the links below. Choose the VPS Starter or Essential plan at OVH, or the HIPAA Compliant VPS at LiquidWeb. Use your clinic's legal name and billing email.
Step 2: Once your account is active, OVH and LiquidWeb will provide you with a root password or SSH login credentials for your new server. These are sent to your account email.
Step 3: Share those credentials securely with your IntakeOS engineer via the encrypted vault our team will set up for you β never by email. Your engineer will use these to install OpenClaw and NemoClaw on your server, then create a separate restricted-access account for ongoing maintenance. Your root credentials will be rotated (changed) immediately after setup is complete, so only your clinic holds the master access.
You do not need to log into or configure the server yourself β that is handled entirely by your engineer. After go-live, you will never need to touch the server again unless you choose to.

Behind the scenes, Twilio routes that call: to your staff first during business hours, and to the AI agent if nobody answers within 3 rings or after hours. Twilio also sends all text messages to patients β booking confirmations, reminders, and rebook offers.

It handles the conversation β asking your qualifying questions, answering patient FAQs, and offering available appointment times. After every call, it automatically sends a full recording and transcript to your patient dashboard so your staff can review what was discussed before the consultation.
β Create account at retellai.comElevenLabs is the voice engine β it determines exactly how your AI intake agent sounds to every patient on every call. The Starter plan ($5/month) is sufficient for most clinics. You choose the communication tone in Section 3, and our setup team selects and configures the right ElevenLabs voice model to match.
ElevenLabs does not connect directly to your IntakeOS system β it connects through Retell AI. Here is how it works: your engineer logs into your Retell AI account, navigates to the voice agent settings, and selects an ElevenLabs voice model from a built-in library inside Retell. Retell and ElevenLabs have a direct platform integration β no separate API key is needed from your side. The only thing required from your clinic is an active ElevenLabs account, which allows the voice library to be used. Once the voice is selected, everything runs automatically. You will hear the voice on your test call before go-live and can request a change if needed.
Create an ElevenLabs account and confirm it is active. That is all that is required from you. Do not worry about selecting a voice or configuring any settings β your engineer handles all of that inside Retell AI during setup. You do not need to provide an API key for this platform.
SendGrid is the service that sends all automated emails on behalf of your clinic. These include: booking confirmations to patients, 48-hour and day-of appointment reminders, a Pre-Consultation Brief to your staff before each appointment (so they know who is coming and what they asked about), attendance confirmation requests, and monthly performance summaries. All emails come from your clinic's own email address β not from a generic sender.
β Create account at sendgrid.comAfter every patient call, Claude reads the full conversation transcript and writes a Pre-Consultation Brief β a plain-English summary that lands in your staff's inbox before the patient arrives. It captures the patient's procedure interest, the answers they gave to qualifying questions, any financing concerns they raised, and key things to know going into the consultation. This means your doctors and coordinators walk into every appointment already prepared, not starting from zero. For US clinics, Anthropic offers a HIPAA Business Associate Agreement.
β Create account at console.anthropic.comCal.com is the scheduling system IntakeOS uses to check your availability and book consultations in real time. When a patient qualifies during a call or web inquiry, IntakeOS checks Cal.com for available slots and books one immediately β no manual scheduling required. It is included as part of your IntakeOS setup at no extra cost.
Unlike most platforms on this page, Cal.com does not need a separate account that you create and pay for. Your engineer installs Cal.com directly onto your private server as part of the IntakeOS setup. Because it is installed on your own server, it is already inside your system β IntakeOS communicates with it internally, without needing an external API key.
Here is how it works in plain terms: when a patient on a call says "Tuesday at 10am works for me," IntakeOS sends a booking request to Cal.com running on your server. Cal.com confirms the slot, adds it to your calendar, and sends a confirmation back to IntakeOS within 60 seconds β all automatically. Your doctor's availability, consultation durations, and daily booking limits are configured in Cal.com by your engineer based on the information you provided in Section 3.
All booked consultations appear in your GoHighLevel dashboard and trigger an automatic email notification to your staff. No separate login to Cal.com is needed for day-to-day operation.
Cal.com is deployed and managed entirely by your IntakeOS engineer. There is no account to create, no subscription to pay, and no API key to provide.
Billing Setup
IntakeOS charges $150 USD per verified attended consultation. There is no setup fee and no monthly invoice β you are only ever charged when a patient attends a consultation that IntakeOS booked.
Step 1: A patient attends a consultation that IntakeOS booked.
Step 2: 2 hours after the consultation time, you receive a simple email: "Did [Patient Name] attend?" Reply ATTENDED.
Step 3: A $150 USD charge is automatically processed to your card on file via Stripe. You receive a receipt immediately.
Step 4: If the patient did not show up, reply NOSHOW. No charge is made, and IntakeOS contacts the patient to rebook.
Monthly minimum: If zero consultations are attended in a given month, a $100 USD minimum retainer applies to keep your system active.
π Your card details are stored securely by Stripe β the world's leading payment infrastructure, used by Amazon, Google, and thousands of businesses. Arbitrage Claw / IntakeOS never sees your card number. After submitting this form, you will receive a secure Stripe link by email to enter your card details on Stripe's own protected page.
Once you submit, our team will review your onboarding information and send you a secure Stripe payment link within one business day. You will use that link to add your card on file β no card details are collected in this form. Your card is not charged until the first ATTENDED confirmation is confirmed by your team.
